Thank you for inviting David and Carolyn to minister with you! We hope that you find this information helpful. Please review, sign, and return this form. Upon receipt of a signed document, you will receive notification from me that the scheduled visit is confirmed. Should these guidelines conflict in any way with your local procedures/policies, please let me know and we will work to accommodate any conflict.

Thank you for your consideration.

• A minimum honorarium between $?? – $?? This is for a four-day ministry visit.
• A love offering is also to be taken to cover miscellaneous travel expenses, e.g., travel insurance, transport, meals etc.
• Please combine all offerings into a payment made payable to Calvary Bible school, and either deliver it to David prior to his departure or pay it into the bank, details provided.

• Two round trip tickets for David and Carolyn or their assistant.
• Payment for these tickets need to be paid before departure, at his discretion. We will do our best to keep the cost reasonable, keeping it in line with due consideration to the schedule of the trip. This does sometimes increase the cost.
• Due to the possibility of scheduling changes, we do not typically book flights more than 4 weeks in advance.
• All travel arrangements will be made by me and I can be contacted at insertemail@. We will send a copy of the flight itinerary and costs once they have been confirmed.
• Transportation should be provided for David and Carolyn, or assistant, between the airport, hotel, and meeting facilities.

Hotel:

• Provision of hotel accommodations. One room if he travels with his Carolyn, and two if traveling with an assistant.
• If possible, David and Carolyn prefer accommodations at a full-service hotel.  We make it a policy of our ministry that we will not stay in host homes.

Meals:

• We request that meals will be provided for David and Carolyn or assistant by the host church or ministry.

• David and Carolyn prefer to use a wireless lapel microphone.
• We request a master quality digital CD and DVD recording, if possible, of every service that Ray ministers in.  Please send these to the address above.

• Colour photograph in a .jpeg format and a bio will be provided for promotional purposes.
